USA might be considered a representative democracy
However that seem to be enough for no one to have any responsibility
No one in the government is responsible for any decisions, since they have all been passed by a majority vote
"elected by the people"
The president might pass executive orders but the president will only stay in his position for a maximum of 8 years and all his work might be undone as soon as he is replaced
Democracy and responsibility are quite the opposite
