So even though intermitent fasting reduces the heart attack rate for fat people, fat people are more likely to get a heart attack so there is a correlation between intermittent fasting and obesity and heart attack
What if intermitent fasting is harmful to skinny or underweight people or people with a low body fat percent and harmful to people who do not have enough carbohydrate or other nutrients in their diet
Specifically lowering the carbohydrate content might potentially turn the protein in the heart muscle into carbohydrate when the blood sugar goes below a certain amount necessary for the brain to get sufficient carbohydrate

So they probably counted people that start this diet but do not change much their bad diet or they claim they do. But do not follow the diet and die.
Another possibility is
fat people are going to get stressed out in the short run if they do not eat all the time
And the Stress will increase their risk of heart attack in the short run
Even though a diet would decrease it in the long run
If you are not used to fasting and are afraid it can do things to how your heart beat patterns and lungs breathing patterns fast
The mystery is not necessarily solved
It depends on if the study involved assigning people to fast or not fast vs people who voluntarily chose to fast
If they did not voluntarily choose to but were assigned to a control or experiment group than the percent of people with a high body fat percent will be different than if it was looking at people who chose to intermittent fast on their own

They might have ssid if I am only going to eat in a narrow window then I will eat more desserts and fat fried foods that I enjoy
